Inspector Notes
An unannounced monitoring inspection was conducted on 8/16/2022. There were a total of 25 children observed with 3 staff members. The children were observed participating in a gaming camp. Multiple different games were being played by different groups of children. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, medication, emergencies and nutrition. A total of 3 child records were reviewed. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
During this monitoring inspection, a self-reported incident received by the department involving the area of care and protection was investigated. Interviews were conducted with staff, documents were reviewed and child records were reviewed. The information gathered during the self-report inspection determined violations with applicable standards or law. A violation notice was given to the center along with this summary. Violations were issued in regards to the self-reported incident.

5Evidence:
On 08/08/2022 child #1 had to be removed from the water by a public attendee at the pool. The child was calling for help, floating on their back in the pool in an area where the water was too deep. The child's paperwork for pool use indicates that the child is a beginner. The child should not leave the shallow end and should not be in water above their shoulders. After the child was helped to the side of the pool by the community member that first heard him calling for help and the camp counselor that was closest to him, local emergency personnel were called to assist with care and transported the child to the hospital. At the time of the incident 3 camp staff members were present at the pool, but not watching the child.
Evidence:
There were 3 staff members supervising at the pool. During the incident the 3 staff members did not have child #1 in sight and sound supervision. Staff #1 stated "they were checking in with a counselor who was on the slide side of the pool. They jumped in the pool to cool off and then came up to finish their conversation. Staff #2 was "helping another child at the time of the incident." and staff #3 was "walking over to that side of the pool because they saw counselor #1 gathered with a group of children." Staff #1 stated that "the child was out of supervision for at least 5 minutes."
