Inspector Notes

An announced, monitoring inspection was initiated and completed on 06/24/2025. The annual staff file review began at 10 am and ended at 2 pm. The inspector reviewed compliance in the areas listed above. The inspector reviewed (5) staff records electronically on 06/24/2025.

This inspection included: document review (i.e. A percentage of staff files; staff qualifications, background checks, certifications, and annual training).

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Staff are to complete the VDOE sponsored online orientation course within 90 days of hire.

A certificate of completion was not available for review for Staff #1.
Plan of Correction: A certificate of completion of the 10-hour preservice training will be obtained and placed on file for future review.

Documentation of at least 2 references to character, reputation, and competency are to be checked prior to employment.

Staff #1, who has been actively working at the center for the past 2 years, had 1 completed reference verification on file for review.
Plan of Correction: A total of 2 complete reference verifications will be obtained and placed on file.
