Staff who have lived outside the state of Virginia within the last 5 years are required to submit background checks from those states. Staff #4 indicated living outside the state of Virginia on their sworn statement of affirmation. The center did not have documentation of a sex offender check or central registry check from that state.
Plan of Correction: The director will send for the required check tomorrow and document in the staff's file.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child Record #4 did not have documentation that the child was adequately immunized by the State Board of Health.
Plan of Correction: The director will follow-up with the parent and request a signed immunization record for child #4.
