Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and an in-person tour of the program. A monitoring inspection was initiated on 1/25/22 after receiving a self-report from the facility in regards to compliance with the regulations.
An in-person inspection at the facility was conducted on 2/10/22 where staff were interviewed, the facility was inspected and documentation was reviewed. The investigation was concluded on 2/16/22. The director was contacted by telephone to initiate the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
