The center is required to have employees undergo a background check every five years. The most recent criminal history check for staff #2 was six weeks overdue.

Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #3 began employment one week prior to receiving completed criminal history check results.

Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for child #1 was not signed.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff #2 only completed 13 of the required 16 annual training hours.

Hazardous substances must be in a locked place that prevents access by children. In a two year old and a three-year-old classroom, there were cleaning solutions on an unlocked shelf, out of reach of children.
Plan of Correction: The facility responded: Hazardous substances were removed and locked. The staff were reminded to lock after use.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. In a two-year-old classroom there was an uncovered outlet, within reach of children.
Plan of Correction: The facility responded: Second eod staff will double check that covers are replaced.

Medication shall be kept in a locked place using a safe locking method that prevents access by children. In the lunchroom, there were three bottles of medication on an unlocked shelf, out of reach of children.
Plan of Correction: The facility responded: Parent was contacted and medication removed. In the future it is brought directly to staff.
Standard 8VAC20-780-70
Staff records shall be kept for each staff person and shall contain all of the required components. The record for Staff #1 did not contain the name, phone number and address of a person to be notified in an emergency. The record for Staff #3 contained only one reference check, two reference checks are required.
