Staff are required to provide a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth. Staff #4 did not have a signed sworn statement of affirmation.
Plan of Correction: This was an oversight. The director will have the staff complete a sworn statement of affirmation and file it in her record.
Standard 22.1-289.035-B-4
The center is required to obtain out-of-state background checks for staff whom have lived outside the state of Virginia within the past 5 years. Staff #10 and #12 indicated living outside the state of Virginia within the last 5 years and did not have background checks for those states.
Plan of Correction: The director will send for the out-of-state checks and document it in the staff member's records. In the future the checks will be sent prior to employment.
