Inspector Notes

Licensing Inspector conducted an unannounced Religiously Exempt Code (Annual/Monitor) Inspection on August 14, 2023. There were 12children ages 3 to 5 years old present under the direct care of 3 staff. The Licensing Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review & discussion with the Director, the Center failed to complete Fingerprint Background Check for all staff prior to the 1st day of employment.

EVIDENCE:
Licensing Inspector reviewed 5 records. Record for Staff #3 did not include documentation of a completed fingerprint background check. Staff #3 1st day of employment was not documented in the record. Sworn Disclosure & Child Protective Services Background Checks & MAT certification were all completed in 2021.
Plan of Correction: The Center must send documentation to the Licensing Inspector that the Fingerprint background check has been requested no later than 10 days following this notification date (August 14, 2023)
