Inspector Notes
An unannounced inspection was initiated on 04/09/2025 completed on 10/07/2025 in response to a complaint received by the licensing office on 04/09/2025 relating to allegations of supervision. The inspector reviewed 5 children?s records and 13 staff records. This inspection included: document review, tour of the facility, interviews, observations and measurements. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Violations
1Staff did not ensure the appropriate supervision of two year old child #1. On 02/07/2025, he was left on the playground when the rest of the class went inside from the playground. Staff reported that child #1 was alone on the playground for approximately three minutes.
