Inspector Notes

A monitoring inspection was initiated on April 18, 2023 and concluded on April 19, 2023.
There were 24 school age children present with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of staff record and interview, the center did not ensure staff obtain and submit the results of a follow up tuberculosis screening every two years.

Evidence: In Staff#1's record the last dated TB screening was 12/21/2020. Interview with staff confirmed the TB documentation available for review during the inspection.
