Inspector Notes

An unannounced monitoring inspection was conducted from 4:00 - 6:00 pm with the director. There were ten children, (eight enrolled in the licensed program), ranging from three years to five years of age, present with three staff supervising. Children were observed playing on the playground, washing hands preparing for snack, eating snack, and playing with age-appropriate toys. Snack served today: pirate booty and mandarin oranges. Four staff records, four child records, two medications, the emergency drill log, daily attendance, written list of food allergies, sensitivities and dietary preferences, and fire and health inspections were reviewed.

Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the violation notice, and then return a signed and dated copy to the licensing office by 4:00pm on Friday, 9/24/2022. Based on observation, it was determined that not all areas outside of the facility were maintained in a safe condition. Evidence:
1) There are cracks in the plastic border maintaining the mulch on the playground that are sharp and potential pinching hazards.
2) The platform on the smaller playground composite structure is rusting and the cover is deteriorating.
Plan of Correction: Not available online. Contact Inspector for more information.

Based on observation, review of the written menu, and an interview with staff, it was determined that the facility did not follow all requirements when providing snacks. Evidence: the written menu listed "Nutrigrain fig bars" for snack, but "pirate booty" and oranges were served. Staff did not update the posted list in the office, or contact parents about the substituted food offered.
Plan of Correction: Not available online. Contact Inspector for more information.
