Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 11/21/2024 and completed on 11/21/2024, as a part of the licensure period. The on-site inspection began at 10:00am and ended at 11:35am. The inspector reviewed compliance in the areas listed above. There were 41 children present and six staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

A facility must not accept a required central registry finding from an applicant, licensee, registrant, or other person required to obtain background checks that is dated more than 90 days prior to the date of employment.

The central registry finding in the record of Staff #5, who has been employed for three months, was dated more than 9 months prior to the date of employment.
Plan of Correction: A new central registry check was requested for Staff #5 on 11/21/24.
