Inspector Notes
A monitoring inspection was completed on March 23, 2023. There were 22 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1Evidence: In Staff#2's record (date of hire: 11/21/2022), the Office of Background Investigation (OBI) determination letter was dated 01/31/2023.
A review of the staff orientation documentation confirmed the date of hire, shadow date (first day on the floor), and completion of trainings.
