The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of Child Abuse and neglect from one state for Staff #1, who has been employed for more than 2 months.

Plan of Correction: January 5, 2026
In response to the violation noted on the report dated 12/10/25, we took the following
action:
Filed the necessary paperwork for the employee noted with Pennsylvania immediately and received the required background check. In the interim, an Aide was in the classroom with the teacher until we received the all clear from the state.
We have returned this plan along with the clearance/background check on January 5, 2026.
