Inspector Notes
A mandated monitoring inspection was conducted on November 27, 2023 from 11:30 A.M.-1:30 P.M., and concluded on December 4, 2023 at 3:30 P.M. There were 61 children present, ranging in ages from two months to five years, with twelve staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations
7Evidence:
1. Staff #4's date of employment was 04/03/2023. There was not documentation of a completed central registry finding in the record.
2. Administration verified that there was not a central registry finding in the record.
Based on review of staff records, the center failed to ensure that documentation of a tuberculosis screening shall be completed at the time of employment and prior to coming into contact with children.
Evidence:
1. Staff # 4's date of employment was 04/03/2023. The tuberculosis documentation was dated 05/01/2023.
2. Administration verified the date of employment and the date on the tuberculosis documentation.
Evidence:
1. In the Toddler Classroom there was a spray bottle of disinfectant spray, and a can of Febreze hanging on the shelf above the diaper changing table.
2. In the Two Year Old Classroom there was a bottle of disinfectant spray hanging on the shelf above the diaper changing area.
3. In the Cafeteria Area there was one bottle of disinfectant spray on a shelf near the entrance to the kitchen, and one bottle of disinfect spray on the shelf next to the storage closet.
Evidence:
1. In the Two Year Old Classroom there four water bottles on the shelf that were not labeled with the child's name, and one water bottle in a cubbie that was not labeled with the child's name.
2. In the Oldest Pre-K Classroom there were four water bottles in cubbies that were not labeled with the child's name.
Evidence:
1. The Licensing Inspector observed children under age of 2 years with "sleep sacks" that include pillows. The children were between the ages of 17-24 months.
2. Staff verified that the children use the sleep sacks on a daily basis and the age of the children.
Evidence:
1. In the Two Year Old Classroom there was a tube of Boudreax Butt Paste that expired in August 2023.
2. In the Toddler Classroom there was a tube of Desitin that expired in September 2023, and a tube of Boudreax Butt Paste that expired in April 2023.
3. Staff verified the dates on the diaper ointments.
Evidence:
1. In the Two Year Old Classroom there was a tube of Blamex Zinc Oxide Diaper Cream that was not labeled with the child's name.
2. Staff verified that the ointment was not labeled with the child's name.
