Inspector Notes

A monitoring inspection was conducted on 05/26/2022 and concluded on 05/26/2022. There were a total of 23 children with 6 direct care staff members present. Children were observed during snack time, water play, and building with large blocks outside. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. The inspection was completed by Ariel Hayes, alongside current Licensing Inspector, Shahana Green. Based on record review, one of the reviewed staff records did not contain the results of a Tuberculosis screening or test every two years from the date of the initial screening or testing, or more frequently as it is recommended by a licensed physician or the health department.
Evidence:
Staff #1 (date of hire 9/2/2014) had a Tuberculosis screening or test dated 9/24/2018.
Plan of Correction: I will find the TB results or have her get a new one.
