During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced inspection was initiated on 3/3/2026 and completed on 3/3/2026 in response to a complaint received by the licensing office on 2/2/2026 relating to allegations of appropriate supervision. The inspector reviewed 5 children?s records and 6 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 3/12/2026. A POC submitted after this date will not appear on the public website.

Standard 22.1-289.031-A-3-c
The required ratio for children ages 24 to 36 months is one staff per eight children. Staff #1 was observed supervising and changing 10 children (youngest child 2 years old) from approximately 10:32am-10:42am, until licensing inspector acknowledged ratio issue and Staff #5 came back into the classroom. Staff #5 stepped out of the room and entered the kitchen to hand out staff paychecks.
Plan of Correction: Provided retraining regarding appropriate staff to child ratios and supervision on 3/4/2026 including written instructions. Provided follow up staff meeting with Director on 3/10/2026.
