Inspector Notes
The licensing inspector conducted an unannounced renewal inspection on Wednesday, May 25th, 2022 from 10:10am to approximately 12:30pm. There were a total of 46 children in care in the direct care of 9 staff members. During the inspection, the children and staff were observed participating in a variety of activities. Staff were observed having positive interactions with the children. All areas of the facility used by the children were inspected. The required postings were reviewed and found to be in compliance. The center is equipped with toys and supplies and items were available to the children. Medication is administered when required and medication and medication authorizations were reviewed. Five children?s records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: 1) The sworn statement in the record for Staff #2, hired on 08/26/19, was dated 01/09/20. 2) The sworn statement in the record for Staff #3, hired on 01/17/22, was dated 01/18/22. 3) The sworn statement in the record for Staff #4, hired on 03/04/22, was dated 03/16/22. An employee or volunteer of a licensed or registered child day program or of a family day home approved by a family day system must not be employed or provide volunteer service until the agency or home has the person's completed sworn statement or affirmation.
Evidence: 1) The license plates for one vehicle are expired. The license plate tags on the vehicle expired in April 2022. 2) During interview, a member of management confirmed the tags were expired and the vehicle was used for transporting children.
Evidence: 1) The record for Child #5, enrolled 03/23/17. did not contain documentation of two emergency contacts. 2) During interview, a member of management confirmed the record was missing two emergency contacts for Child #5.
