Inspector Notes

The department received a complaint incident regarding allegations in the areas physical plant, staffing and supervision on March 10, 2022. A complaint inspection was initiated on March 17, 2022 and concluded on April 14, 2022. The licensing inspector was onsite on April 14, 2022 from approximately 8:45am until 9:45am. Observations were made at the center, interviews with staff were completed and documentation was reviewed.

The evidence gathered during the investigation did not support the allegation of non-compliance with standards or law. Any other violations not related to the complaint but identified during the course of the investigation can also be found on the violation notice.

Based on review of two staff records, the center did not ensure all required background checks were repeated at least every 5 years.

Evidence:
1. The record for Staff #1 (hire date 08/19/1990) contained a central registry finding dated 10/31/2016.
2. A member of management confirmed a repeat central registry finding was not on file for Staff #1.
Plan of Correction: We have requested the central registry update and will add it to the record.
