Inspector Notes

An unannounced monitoring inspection was conducted today from 11:45 am to 1:15 pm. There were 59 children directly supervised by 11 staff. The physical plant, 3 staff records, children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping and reading books. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard 22.1-289.035-B-4
Based on record review and staff interviews, it was determined the center did not obtain a copy of the results of a criminal history record information check, and a search of the child abuse and neglect registry for a staff member from the out of state where the person resides.
Evidence:
1. Staff #2 who was hired on November 15, 2022 lives outside of the Commonwealth.
2. The Center has sent request but staff member has to go to have the clearance completed but the earliest appointment available is mid January.
Plan of Correction: We have worked to schedule appointment but the agency stated they were well over a month behind. In the future, we will set the appointment immediately.
Standard 8VAC20-780-270-A
Based on observation, it was determined area outside was not maintained in a safe condition.
Evidence:
1. On the playground, the center has plastic lawn edging and there were places where the bolts holding it into the ground were not flush. This poses a protrusion and/or snag hazard.
Plan of Correction: We put in a work order and we will ensure children stay away from the edging until fixed.

Based on observation, it was determined that not all occupied cots met 12 inches of space between them.
Evidence:
1. In the preschool room, there were 4 occupied cribs that were close together and not 12 inches apart.
Plan of Correction: We were willing to correct at the moment but will make sure all cots stay 12 inches apart.
