Inspector Notes

An unannounced inspection was initiated on 11/15/2024 and completed on 11/15/2024 in response to a complaint received by the licensing office on 11/8/2024 relating to allegations of forbidden actions. The inspector reviewed 1 staff record on-site. This inspection included, document review, tour of the facility, and interviews. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to Complaints and Reports of Suspected Child Abuse or Neglect and are documented on the violation notice issued to the program

Staff employed by a child day program who have reason to suspect that a child is abused or neglected shall report the matter immediately to the local department's toll-free child abuse and neglect hotline.

Staff became aware of a reportable offense, and did not report it to the local department for 6 days.
Plan of Correction: The center will immediately report any suspected abuse to the child abuse and neglect hotline.
