During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 7/16/2025. The on-site inspection began at 12:55pm and ended at 2:25pm. The inspector reviewed compliance in the areas listed above. There were 53 children present and 13 staff. The inspector reviewed five children?s records and five staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Child #1, who had been attending for ten months, did not have documentation of their 18 month immunizations.
Plan of Correction: The center will obtain the updated immunization records for the child.
Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There were several fence boards that were broken at the top or needed to be secured in place.
Plan of Correction: The center will have the fence boards repaired.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #1, enrolled for ten months, was missing the work phone number of one parent who has custody. The record for Child #2, enrolled for one month, was missing the work phone number of both parents who have custody.
