Inspector Notes
An unannounced monitoring inspection was initiated on and completed on 11/7/2025 in response to a self-report received by the licensing office on 11/5/2025 relating to child abuse and neglect. The inspector reviewed one staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
