The center is required to have employees undergo a background check every five years. The most recent central registry check for staff #4 was overdue by fourteen days. The most recent out of state central registry check for staff #4 was overdue by one day.
Plan of Correction: The center will have the staff complete an updated central registry search for Virginia and the state they have resided in the previous five years.
Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Caregiver #1, who had been employed for longer than two years, did not have a completed national criminal background check. A completed national criminal background check was found in the caregiver's file, but the name searched was misspelled.
Plan of Correction: The center will have the staff complete a new criminal background check with the correct spelling.
Standard 8VAC20-780-420-E-3
Staff shall request at least annually parent confirmation that the required information in the child's record is up to date. Up-to-date information is required to be requested from the parent for each child's record at least annually. The information in the record of child #2 had not been updated in over eighteen months, which is overdue by six months.
Plan of Correction: The center will have the parents update the emergency contact information.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child #2 and #4, enrolled, was missing the work phone number of one parent who has custody.
Plan of Correction: The center will obtain the work phone numbers for the parents.
Standard 8VAC20-780-90-A
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreement shall be signed by the parent and include a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. The records for child #4, did not contain this written agreement.
Plan of Correction: The center will have the parents complete the parental agreement.
