Inspector Notes

An unannounced monitoring inspection was conducted on 3/22/2024 between the hours of 9:00am-11:00am. There were 37 children directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, and injury reports were inspected. Children were observed playing outside and having Easter-themed snacks. Handwashing was also observed. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-160-A
Based on record review, 1 of 5 staff reviewed did not submit documentation of a negative tuberculosis screening completed within the last 30 calendar days of the date of employment.

Evidence:
1. The tuberculosis screening in the record for Staff #1 (employment date 3/15/2024) was dated 1/24/2024.
Plan of Correction: The staff member will obtain a new tuberculosis screening. Tuberculosis screenings for new staff will be obtained within required time frames.

Based on record review, 1 of 2 written records of injuries did not contain all required information.

Evidence:
1. An injury report for Child #4 dated 09/08/2023 did not contain the time when parents were notified, and the documentation on how parent's were notified.
Plan of Correction: The center will make sure all information is documented on the injury report form.
