Inspector Notes

An unannounced inspection was initiated on 11/21/2024 and completed on 12/05/2024 in response to a complaint received by the licensing office on 11/15/2024 relating to allegations of staffing and supervision and special care provisions and emergencies. The inspector reviewed 1 child?s record on site. This inspection included, document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid.

Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The center shall maintain a written record of children?s serious and minor injuries in which entries are made the day of the occurrence.

Staff 2 confirmed that on 11/14/2024, Child 1 had a visible mark near their eye at the time of their departure from the center that was not there at their arrival. Staff 2 confirmed that there was not a written record of the minor injury.
Plan of Correction: The incident has been documented. Working with the Health Advocate to be more aware of any injuries. Retraining and reviewing documenting with the staff.
