Inspector Notes

An unannounced monitoring inspection was initiated on 03/04/2025 and completed on 03/13/2025 in response to a self-report received by the licensing office on 03/04/2025 relating to programs. The inspector reviewed 1 child?s record and 1 staff record on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

Staff 1 pushed two children apart in efforts to separate the two children, resulting in Child 1 (5 years old) hitting the back of their head on a retaining wall on the outdoor playground. Staff 1 got close to the child and pointed their finger in the child?s face.
Plan of Correction: The staff member was terminated. There will be an increase in training and available resources.
