This inspection was conducted by licensing staff using both an alternate remote protocol, including telephone contacts, documents reviewed, review of video footage and interviews .
An self report investigation was initiated on September 22, 2021 and concluded on October 15, 2021. The investigation was in the area of programs. The director was contacted by telephone, interviews were conducted, video footage was reviewed.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
