The licensing inspector initiated joint investigation with local agency in response to a serious injury/self report . A unannounced non-mandated inspection started on June 6, 2022 in regards special care provision and emergecnies . Review of video footage, center documentation, staff interviews . Based on information gathered during the investigation there was no violations.
