An unannounced complaint investigation was conducted on 8/12/2022 regarding administration, programs, staff special care provision and emergency, staffing and supervison and staff qualifications and training. Observation and interviews were conducted, and records were reviewed. There was not a preponderance of evidence to support the allegations and the complaint has been deemed not valid.
