Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/30/2024 and completed on 07/30/2024. The on-site inspection began at 12:00pm and ended at 2:00pm. The inspector reviewed compliance in the areas listed above. There were 105 children present and 17 staff. The inspector reviewed 10 children?s records and 8 staff records on-site. This inspection included document review,
tour of the facility, interviews and, observations

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on August 13,2024. A POC submitted after this date will not appear on the public website.

Based on staff interview and documentation review, the licensee did not ensure annually approval from the health department be provided for meeting requirements for water supply; sewage disposal system and food services.

Evidence:
The center provided food establishment inspection but did not provide the water supply and sewage disposal system inspection (Enviromental Sanitation inspection ).
Plan of Correction: Enviromental Sanitation inspection report was obtained 7/30/2024. School management will request the inspection report to be completed each time health inspection completes food establishment inspection .

Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean safe and operable condition.

Evidence:

In the Infant classroom two, the door to enter the classroom would not open properly . The door was sticking when trying open and close.

In the infant classroom two, the shelves for storing belongings had two drawers with the bottom corners cracking causing rough and sharp edges.

Plan of Correction: All repairs were made by 08/02/2204. Moving forward, staff will complete daily observations of equipment and report to management when repairs are needed. School Management will audit classrooms weekly to ensure all equipment is in good repair.
