Inspector Notes

An in-person monitoring inspection was conducted today. There were 3 groups of children present (26 children) with 3 staff. Violations were discussed with the Center Supervisor (CS) and a plan of correction was provided to the LI. Medications, emergency evacuation drills, emergency supplies and first aid supplies are all in compliance. Children were observed participating in a variety of activities after school.

Based on a review of records, one child record did not contain documentation of a completed physical within 30 days of enrollment.
Evidence:
1 of 3 records reviewed did not contain a physical. (Date of Enrollment: 12/10/21) (Child 1)
Plan of Correction: I will pick up the physical from the front office tomorrow.
Standard 8VAC20-780-280-B
Based on direct observation hazardous substances were not kept locked.
Evidence:
Room 1 had hand sanitizer in an unlocked cabinet.
Room 2 had isopropyl alcohol, hydrogen peroxide, saline solution and shaving cream in an unlocked cabinet.
Plan of Correction: All products were moved to a locked location during the inspection.

Based on a review of records, one child record did not contain all required components.
Evidence:
1 of 3 records reviewed did not contain an allergy action plan with physician's instructions for a diagnosed food allergy. (Child 2).
Plan of Correction: The parent was given the DOE model form at pick up today to be completed.
