Inspector Notes

A monitoring inspection was initiated on 3/3/2023 at approximately 9:00am and concluded on 3/3/2023 and approximately 9:45am. There were a total of 15 children with 2 staff members present. Children were observed during playing board games, coloring, and engaged in group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on file review and discussed with center staff, the center did not maintain and keep record of the work phone number of each parent who has custody of each child enrolled.

Evidence: There was no record of the work phone number for the parents of child #1 and child #2.
Plan of Correction: Center will contact parents of both children and obtain their work phone numbers.
