During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 7/11/2025 and completed on 8/1/2025 in response to a self-report received by the licensing office on 7/08/2025 relating to supervision. The inspector reviewed 2 children?s records and 1 staff records on-site on 7/11/2025. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 08/08/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On multiple dates between June 24 and July 2, there was alleged inappropriate physical contact between two children.
Plan of Correction: Immediately upon notification of the alleged behaviors between two children, on-site supervisors informed all center staff and immediately put additional measures in place to ensure their care, protection, and guidance.
Standard 8VAC20-780-420-B
Staff shall promptly inform parents when persistent behavioral problems are identified.

Staff had knowledge that two children had engaged in alleged inappropriate physical contact as of 7/8/2025, however, as of 07/22/2025 the parent of Child #2 had not been notified of the behaviors.
Plan of Correction: Staff will ensure parents are informed promptly. The parent of child #2 returned to the center on July 23rd, 2025, after having been out of town. Staff met with the parent in-person to inform them of the alleged behaviors.
