Inspector Notes

An unannounced monitoring inspection was initiated on 07/16/2025 and completed on 07/16/2025, with local agencies, in response to a self-report received by the licensing office on 07/14/2025 relating to Administration, Staff Qualifications & Training, Staffing & Supervision, and Programs. The inspector reviewed children?s records on-site, staff records on-site and additional staff records electronically. This inspection included document review, a tour of the facility, multiple interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 7/28/2025. A POC submitted after this date will not appear on the public website.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for approximately 6 weeks, did not have a central registry search requested prior to their start date.
Plan of Correction: A Central Registry Search will be requested prior to an employee's first day of employment including prior to participation in orientation and training. Administrative staff will follow-up with any requests not received.
