Inspector Notes
A monitoring inspection was conducted in person on March 14, 2022 There were 45 children present, ranging in ages from 5 to 12 years, with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of four children's records, the center did not maintain a record for two child that contained two emergency contact persons for each child.

The record for child #1 did not contain two emergency contacts that included the name, address and phone number of two people to contact in case a parent was not available, only one was listed.
