Inspector Notes

A monitoring inspection was conducted on 04/28/2022. There were 13 children present, ranging in ages from 5 to 12 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 1) On the date of inspection, the results of the health department's annual inspection were not available for review. 2) Staff #1 inquired with the school administration but a copy of the report was not available.
