During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 10/02/2025. The on-site inspection began at 1:30 pm and ended at 3 pm. The inspector reviewed compliance in the areas listed above. There were (23) children present and (4) staff. The inspector reviewed (5) children?s records and (3) staff on-site documentation on-site on 10/02/2025.

This inspection included: document review (i.e. Files, injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms); tour of the facility; interviews; and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Physical examinations completed by a physician shall be obtained within 30 days after the first day of attendance.

There was no physical examination in the record of Child #3, who had been enrolled for over 1 year..
Plan of Correction: The physical for Child #3 will be obtained and placed on file.
Standard 8VAC20-780-550-G
Documentation shall be maintained for all shelter-in-place drills that are conducted.

The shelter-in-pace drill conducted in the Spring of 2025, was not recorded in the post drill log.
Plan of Correction: This drill will be recorded. Moving forward, all conducted drills will be noted in the drill log on the day they were practiced.
