Inspector Notes

An unannounced monitoring inspection was initiated and concluded on 10/24/2022. There were 27 children present with 3 staff supervising. The children were observed building a large structure with blocks, playing word games, working on class art project and handwashing when returning from the gym. The physical plant, children's records, staff records, menu's, evacuation logs, attendance logs, medication and medication authorizations were reviewed. Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
Child #2, #3 and $3's records to not contain the work phone number of each parent who has custody.
Plan of Correction: I will ask parents to provide information and will add it to children's record.
