Inspector Notes

An unannounced renewal inspection was conducted on 11/1/2023 between the hours of 4:30 and 6pm. There were 33 children supervised by 3 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in small group activities, having snack, and playing outside. Areas of non-compliance are identified in the Violation Notice.

Based on observation, hazardous substances were not kept in a locked place.

Evidence:
Cleaning wipes were observed on the refrigerator, accessible to children.

Based on review of medications,

Evidence:
1. Two medication authorizations for Child #5 expired on 7/11/2023. The medications have not been picked up by the parent.
2. A medication authorization for Child #6 expired on 08/18/2013. The medication was not picked up by the parent.
Plan of Correction: All medications with expired authorizations will be picked up by parents.

Based on record review, 3 of 4 children's records did not contain all required information.

Evidence:
1. The records for Child #2, #3, and #4 did not contain the phone number for each parent's place of employment.
Plan of Correction: The center will have the parent write the required information on the form.
