Inspector Notes

An unannounced monitoring inspection was conducted today from 12:40pm- 2pm. There were 42 children directly supervised by 3 staff. The physical plant, 3 staff records, 4 children?s records, medications, evacuation drills, and emergency supplies were inspected. Children were observed participating in group play, reading books, and making crafts. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-130-A
Based on record review, the center did not obtain documentation of immunizations before the child's first day for 1 of 4 children reviewed.

Evidence:
1. The record for Child #1 (start date 8/23/2021) did not contain documentation of immunizations.
Plan of Correction: Will follow up with the parents or request document from the school office by the end of the week.
Standard 8VAC20-780-140-A
Based on record review, the center did not ensure that 1 of 4 children reviewed had a physical examination within 30 days of the child's first day.

Evidence:
1. The record for Child #1 (start date 8/23/21) did not contain documentation of a physical exam.
Plan of Correction: Will follow up with the parents or request document from the school office by the end of the week.
Standard 8VAC20-780-350-C
Based on observation, staff-to children ratios were not followed.

Evidence:
1. Upon arrival at 12:40pm, there were 42 children present, ranging in age from 6 to 12, with two staff members supervising.
2. Based on ratio requirements for the mixed age group present, the ratio required is 1 staff member per 18 children.
3. The center was observed to be out of ratio from time of arrival (12:40pm) till 1:05pm when another staff member arrived.
Plan of Correction: The center will establish a plan for increased numbers.
Standard 8VAC20-780-420-E-3
Based on record review, staff did not request at least annually, parent confirmation that the required information in the child's record is up to date.

Evidence:
1. The most recent parent confirmation that Child #2's record is up to date was dated 8/21/2021.
Plan of Correction: Will follow up with the parents to review the information.
Standard 8VAC20-780-60-A
Based on record review, 2 of 4 child records reviewed did not contain all required information.

Evidence:
1. The record for Child #3 and Child #4 did not contain documentation of the 1st date of attendance.
2. The record for Child #4 did not contain the phone number for one parent's place of employment
Plan of Correction: Will follow up with the parents to request the information.

Based on record review and staff interview, staff records were not kept for 1 of 3 staff members reviewed.

Evidence:
1. The center did not have any documentation for Staff #3.
2. Staff #3 reported she was substituting at the center and did not bring the required documentation with her.
Plan of Correction: Staff is providing support as a substitute and will bring the information tomorrow.
