Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On December 30, 2025, Child #1 (age: 5 years) was left on the playground, unsupervised. Child #1 exited the playground and walked to the front of the school where they were let into the building by Staff #1. The playground and sidewalk leading to the front of the school are located next to the school?s parking lot.
Plan of Correction: Staff will always ensure the care, protection, and guidance of children, and will follow all program procedures to ensure the safety and accountability of all children.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On December 30, 2025, Child #1 (age: 5 years) was left on the playground, unsupervised. Child #1 exited the playground, walked to the front of the school, rang the doorbell and was let into the building by Staff #1. Child #1 was out of sight and sound supervision of staff for approximately 2 minutes.
Plan of Correction: Staff will ensure children are within sight and sound supervision at all times, and will follow all program procedures to ensure the safety and accountability of all children.

The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with the center's own policies and procedures that are required by these standards.

The center has a supervision policy that states ?each staff will account for the children in their care on their checklist before an individual child or a group transitions to a specific location,? ?be clear about the role each staff is responsible for during transitions,? and ?two staff should be responsible for seeing that the transition flows smoothly. One should initiate the transition by leading the group into the new area. The other makes sure that all the children reach the new area and none are left behind.?

On December 30, 2025, seven staff transitioned a group of children from the playground to the classroom. Child #1 (age: 5 years) was left on the playground, unsupervised. Multiple staff reported that the children were not accounted for on the accountability sheet before they transitioned to the classroom. Staff also stated that the role each staff was responsible for during the transition to the classroom was not clear. During the transition, no staff made sure that all of the children reached the new area and none were left behind.
Plan of Correction: Staff will follow all program procedures for safety and accountability in order to ensure the care, protection, guidance and sight and sound supervision of all children. Program and center specific safety and accountability procedures will be reviewed with all Clermont spring program staff prior to the spring break full day program to include review of staff roles and responsibilities, playground supervision, accountability checklists, transitions, communication between staff, and review of procedures with all children. Safety and accountability procedures and expectations during full day programs will be reviewed with all staff program wide prior to the spring & summer programs.
