Inspector Notes

Conducted an investigation into a self-reported supervision incident. Staff interviewed and records reviewed. Based on the information gathered areas of non-compliance with standards and laws reviewed were found. Questions about this investigation may be directed to pamela.sneed@doe.virginia.gov

Standard 8VAC20-780-340-A
Based on interviews conducted and records reviewed, staff failed to ensure the care, protection and guidance of a 5 year-old child.

Evidence:

1. During a fieldtrip on 4/14/23, a 5 year-old child was out of sight and sound of staff for approximately 6-8 minutes.
2. The 5 year-old child, Child #1, was left inside the facility while other children and staff in the group had exited the building and were getting onto the school bus to depart.
3. Staff #1 had been told by other staff to walk at the rear of the group of children and to hold the hand of Child #1, as the child had been "wandering off."
4. Staff #1 stated "[name of Child #1] got distracted at the gift shop" while the group was exiting the facility.
5. Staff #1, who was assigned to the group of 12 children, returned to the school bus with 11 children and was unaware that Child #1 had been left behind.
6. Child #1 was found by staff inside the facility with an unknown "family" and appeared to be "sad and crying."
7. All staff interviewed described the facility as being "crowded" and "busy."
Plan of Correction: Staff will ensure the care, protection, and guidance of all children through the consistent implementation of SACC's safety and accountability procedures including while on field trips and during times of transition. The Regional Supervisor reviewed program safety, supervision, and accountability procedures with Bren Mar Park staff. In addition to the center specific training, all staff working the summer program participated in active supervision training including safety and accountability procedures for field trips. Procedures for ensuring sight and sound supervision, planning for children's specific needs, adapting to the environment, as well as accounting for all children before and after transitions and conducting periodic name to face roster checks were emphasized. The date for a follow up meeting with all Bren Mar Park staff is on 6/14/23 and the date to review active supervision with summer staff is on 6/21/23.

Based on interviews conducted and records reviewed, staff failed to ensure that a child under the age of 10 years-old was always within actual sight and sound supervision of staff.

Evidence:

1. During a fieldtrip on 4/14/23, Child #1, a 5 year-old child was out of sight and sound of staff for approximately 6-8 minutes.
2. Upon exiting the indoor facility, Staff #1, who was assigned a group of 12 children to supervise, returned to the school bus with 11 children and was unaware that one child had been left inside the facility.
3. Staff #2 counted the number of children in Staff #1's group and determined that one child was missing, and proceeded to identify the missing child to be Child #1.
4. It took staff approximately 3-5 minutes to locate Child #1 in the facility and return to the bus.
Plan of Correction: Staff will consistently implement SACC's safety and accountability procedures at all times including while on field trips and during times of transition in order to ensure all children are always within sight and sound supervision of SACC staff. The Regional Supervisor reviewed program supervision and accountability procedures with Bren Mark Park staff. In addition to the center specific training, all staff working the summer program participated in active supervision training, including safety and accountability procedures for field trips. Procedures for accounting for all children before and after transitions and conducting periodic name to face roster checks were emphasized. The date for active supervision training was on 4/26/23 where all of the above protocols were reviewed. The date for 1 on 1 follow up meeting with the head teacher was on 5/19/23.
