Inspector Notes

Conducted an unannounced inspection at 9:25am. Observed 16 children + 2 direct-care staff. upon arrival. Additional staff and children arrived during the inspection. Ratios and supervision were in compliance. Children were engaged in a variety of activities to include drawing, playing with toys and tablets, and outdoor active play. Staff were actively engaged with the children and activities. Children brought their own food for lunch today. The facility was found to be clean and well supplied toys and equipment for the children. An area of non-compliance with standards and laws reviewed was found. Based on records reviewed, 1 of 2 children's records did not include documentation of immunizations before the child attended. Evidence: The record for Child #1 did not include the immunization record.
Plan of Correction: The Center Supervisor said the immunization record will be obtained from the school office/nurse.
