Inspector Notes
An unannounced, on-site SHSI monitoring inspection was initiated and completed on September 10, 2024, as a part of the licensure period. The on-site inspection began at 3:15pm and ended at 4:00pm. The inspector reviewed compliance in the areas listed above. There were 4 children present and 4 staff. The inspector reviewed 4 children?s records, and 3 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
