Inspector Notes

An unannounced monitoring/renewal inspection was conducted today from 10:15 am-11:10am. There were 19 children directly supervised by 2 staff. The physical plant, staff records, children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing on the playground. Areas of non-compliance are cited on the Violation Notice.

Standard 8VAC20-780-550-G
Based on a review of evacuation logs, the center failed to document emergency evacuation, shelter in place and lockdown drills.

Evidence:
The Director stated that they had completed an evacuation drill; however, there was no documentation of the drill.
Plan of Correction: I will ensure that all evacuation drills will be documented.
Standard 8VAC20-780-60-A
Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.

Evidence:
- Child #3 and #4's records are missing documentation of a parent signature on the Health Certificate.
- Child #1's record is missing both parents workplace.
- Child #5's record is missing both parents workplaces and work phone numbers.
- Child #3's record does not contain a written care plan for a child diagnosed with a food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reations.
Plan of Correction: I will get information from parents and place in children's records.
