Inspector Notes
An unannounced monitoring/renewal inspection was conducted today from 10:15 am-11:10am. There were 19 children directly supervised by 2 staff. The physical plant, staff records, children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing on the playground. Areas of non-compliance are cited on the Violation Notice.

2Evidence:
The Director stated that they had completed an evacuation drill; however, there was no documentation of the drill.
Evidence:
- Child #3 and #4's records are missing documentation of a parent signature on the Health Certificate.
- Child #1's record is missing both parents workplace.
- Child #5's record is missing both parents workplaces and work phone numbers.
- Child #3's record does not contain a written care plan for a child diagnosed with a food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reations.
