Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and an in-person tour of the program.
A monitoring inspection was initiated on 4/5/2022 and concluded on 4/5/2022. There were 30 children present with 13 staff supervising. The inspector reviewed compliance in the areas of physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There were no medications on site. A total of 3 child records and 4 staff records were reviewed. Children at the program were on Spring Break. They were observed in the gym during gross motor activities.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on an interview with the Center Supervisor, documentation was not available to reflect any evacuation drills. Evidence: The Center Supervisor could not find the Evacuation Drill logs to demonstrate the fire, shelter-in-place and lockdown drills had been completed.
Plan of Correction: I will create a new document & file it & tell other staff where the sheet is filed.
