Inspector Notes

An in person renewal study was conducted today after school.
There were 36 children preset with 3 staff. Emergency evacuations drills, first aid supplies, emergency supplies, medications, menu, child records are all in compliance.
Staff records are reviewed for specific items only as the full record is reviewed at another time in the main office.
Daily Health Observation training violation was discussed with the supervisor.
Additional violations were found during the course of processing the application for renewal.
The report will be typed off site and emailed for signature.

*Based on a review of records, one staff record does not contain documentation of a competed Child Abuse and Neglect search for a residence outside of Virginia in the past 5 years, within 45 days of employment.

1 staff record listed Louisiana as a residence in the past 5 years. There is no completed Criminal Record Check or a completed Child Abuse and Neglect search available for review.

Based on a review of documentation, one staff record did not contain documentation of a completed Sworn Disclosure Statement (SDS) prior to the first date of employment.
Evidence:
1 staff record contained an SDS that was dated after the documented, first date of employment. The SDS available for review is dated 8/29/2022.
