During the inspection, the inspector reviewed the areas. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/5/2025, as a part of the licensure period. The on-site inspection began at 7:20am and ended at 8:10am. The inspector reviewed compliance in the areas listed above. There were 14 children present and 4 staff. The inspector reviewed 4 children?s records and 2 staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 9/15/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-240-E
Within 30 days of the first day of employment, staff must complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Staff #1 has been employed for over 90 days and did not have proof of completing CPR or First Aid certification. Staff #1 was alone with children outside.
Plan of Correction: Director in contact with Administration to see when staff certificates will arrive.
Standard 8VAC20-780-540-C
The required first aid kits shall include at a minimum,triangular bandages.

The fist aid kit only had one triangular bandage.
Plan of Correction: Director located second triangle bandage. They will add to first aid kit .
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain name, address, and phone number of two designated people to call in an
emergency if a parent cannot be reached.

Child's #4's record did not have addresses for emergency contacts.
Plan of Correction: Staff will reach out to parent, to obtain information and correct licensing form.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

The attendance record for 9/5/2025 marked all children children as having departed, while still in attendance.
Plan of Correction: Director will train staff on the correct way to do morning attendance.
