Inspector Notes

An unannounced monitoring inspection was conducted today from 8:15- 9:05. There were 16 children directly supervised by 2 staff. The physical plant, 4 staff training records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, preparing for a Pokeman Tournament, making banana beignets from scratch, and eating the treats. There was an assortment of age appropriate materials for the children in care. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard 8VAC20-780-40-E
Based on record review, it was determined the center is not ensuring the activities and services are in compliance with its own policies and procedures.
Evidence:
1. Child #1 has been enrolled since early January with long term medication needs. The Centers policies state long term medication requires physician authorization.
2. Center does not have written long term authorization.
Plan of Correction: We will have parent keep filling out a new medication administration form until the parents- obtains the physician's signature.
Standard 8VAC20-780-550-P
Based on record review of an injury report, the center did not complete the report.
Evidence:
1. An injury report from January 25, 2023 lists the staff who wrote it up and this staff was same who witnessed the accident.
2. This staff member did not sign, did not get a second staff member signature to verify the parent was notified at the end of the day.
Plan of Correction: We will review with staff again about properly filling out the reports.
