Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 9/23/2025. The on-site inspection began at 2:30pm and ended at 3:20pm. The inspector reviewed compliance in the areas listed above. There were 34 children present and 4 staff. The inspector reviewed 5 children?s records and 4 staff records on 9/23/2025. This inspection included: document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-P
The center is required to maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include staff and parent signatures or two staff signature.

An incident/injury report reviewed from the month of September did not include any staff or parent signatures on it.
Plan of Correction: We will make sure we include the required signatures on all incident/injury reports moving forward.
